Heartland Healthy Neighborhoods is now working in partnership with the Kansas Foundation for Medical Care (KFMC) to improve health outcomes and care delivery for Topeka and Shawnee County Medicare Consumers. With extensive support from KFMC, a Communities of Care group is being established under the Heartland Healthy Neighborhoods umbrella. This group will work to reduce unnecessary hospitalizations, reduce hospital readmissions, reduce the number of Adverse Drug Events (ADEs) and improve the ability of Medicare consumers to age in place.

 

Meetings will take place monthly on the first Wednesday of each month and be held at the Topeka and Shawnee County Public Library at 3:00 pm. Any organization, agency, family, direct-Medicare consumer, and healthcare based organization/representative is invited to attend.
